Statement of Need
The findings from the Komen North Jersey 2011 community profile revealed that two specific populations are significantly medically underserved African Americans and the Latino Community. The priorities for the 2013 FY RFA are listed below:
Drawing from the Komen North Jersey 2011 community profile and subsequent research the Affiliate has identified the following funding priority areas:
1. Breast Cancer Screening, Diagnosis and Treatment Assistance (defined as one or more of the following) screening, diagnosis, treatment support/assistance and navigation. 75% of the affiliate grant funds will be allocated to these grants.
Any patient that receives a screening or diagnostic test paid for by Grant Funds must meet the New Jersey State Department of Health and Senior Services (“NJDHSS”) guidelines attached hereto for either Cancer Education and Early Detection (NJCEED) or Charity Care Assistance as they may be in effect from time to time.

2. Education (defined as one or more of the following) education and survivorship programs. 25% of the affiliate grant funds will be allocated to these grants
3.Target populations African Americans and Latino Community
4.Applicants must provide service in one of our nine counties: Bergen, Essex, Hudson, Morris, Passaic, Somerset, Sussex, Union and Warren.
•Please note, if your grant in the past combined both education and screening/treatment assistance, you must submit two separate grants this year.
